• Summary: Lack of Human Oversight – The teacher relied on an AI-generated unit plan and missed that some critical safety standards were omitted. This happened because there wasn’t sufficient human oversight to catch the AI’s gaps in content. Humans did not adequately supervise or verify the AI’s work.

The teacher (or an instructional coach) needs to thoroughly check the AI-generated lesson plan against official requirements and standards. Instituting a rule that any AI-created curriculum content undergoes a manual compliance check would prevent such oversights.

• High Risk – Missing essential safety guidelines in a construction safety module is a serious issue. It could lead to students not learning critical safety practices, which poses legal liabilities and potential physical danger in the future. The severity is high because it concerns compliance and student safety.
   

• Decision: The risk of overlooking safety standards is too high to accept. The council would mitigate by ensuring robust human review of AI outputs. For instance, after generating a lesson plan with AI, the teacher must cross-verify it with safety standards and school curriculum requirements. By mitigating – perhaps through training teachers to treat AI suggestions as drafts that require verification – the school maintains compliance and student safety, rather than gambling on the AI getting everything right.
